This was not met as evidenced by: During a tour of the facility, LPA determined S1 does not have a criminal record clearance. This poses an immediate threat to the health and safety to the persons in care.

    (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This was not met as evidenced by: During a tour of the center, LPA noted areas of the center not being used with signs of rodents. Additionally observed were cobwebs, dust, debris, rugs, large toys, and stacks of paper piled on top of each other. LPA noted a foul odor in one of the on-limits classrooms, & unable to determine its source.
