OUTDOOR ACTIVITY SPACE
Based on observation, the licensee did not comply with the section cited above. The outdoor drinking water fountain, has two wooden pieces that the fountain is bolted on. One out of the two pieces has completely worn down, exposing sharp pieces of wood. The water fountain itself works, and has collected dirt and leaves on it, as well as calcium from water. This poses/posed a potential health, safety or personal rights risk to persons in care.

The kitchen door always remains open, for ventilation purposes, children do access the area near the kitchen door. Furthermore, there is a drawer with kitchen knives also accessible. Additionally observed were cobwebs, dust, debris, rugs, large toys, and stacks of paper piled on top of each other. LPA noted a foul odor in one of the on-limits classrooms, & unable to determine its source.
