Inspector’s narrative

What the inspector wrote

a staff hold and pull a child by the hand/arm and/or lift up children under their arms to carry to the bathroom. Based on information gathered from interviews and records, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. See LIC9099D for deficiency cited. An exit interview was conducted, a copy of this report and Notice of Site Visit was provided to the facility representatives, Araceli Rios, Director and Teresa Addison, Regional Manager. LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS On 04/01/25 LPA reviewed samples of video footage dated 2/21/25 and 2/24/25 including footage from alleged day/time of incident. LPA observed videos reviewed do not have audio and is unable to determine if yelling occurred. Due to conflicting information obtained from what was alleged, the evidence collected was not sufficient to substantiate or refute the above allegation. Although the allegation may have happened, or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. An exit interview was conducted, a copy of this report and Notice of Site Visit was provided to the facility representative. LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS
