    101238.2(d)(2) Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris... that pose a... hazard.This was not met as evidenced by: A box/carton containing multipe razor blades was discovered by children in care, resulting in two injuries. Staff stated they conducted a safety check of outdoor playspace prior to incident but did not observe any hazards. This is an immediate health safety and personal rights risk to children in care.
