What the inspector wrote

On date and time listed, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: \342\200\242 Normal days and hours of operation are: 6am - 6pm \342\200\242 The off limit areas include: garage, dining room, and all of the upstairs. \342\200\242 The facility is licensed to have no more than 14 children as a large FCCH and is operating within the licensed capacity and appropriate ratios. An assistant provider is required to have 14 total children. Without an assistant provider present, the maximum capacity is 8 total children. \342\200\242 Appropriate supervision was being provided during this inspection \342\200\242 A working telephone is present, and the current phone number is on file \342\200\242 A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection, both were not currently functional. However, no children enrolled at this time or for the last 5 years per the licensee. SEE LIC9102TA for Advisory Note issued. This is not a deficiency. \342\200\242 Fireplace is properly screened to prevent access by children \342\200\242 All hazardous items NOT are stored inaccessible to children. This is not a deficiency. \342\200\242 Toxins are NOT locked and inaccessible to children in care. However, no children enrolled at this time or for the last 5 years per the licensee. SEE LIC9102TA for Advisory Note issued. This is not a deficiency. \342\200\242 Weapons are not present per the licensee. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \342\200\242 Stairs are barricaded \342\200\242 Clean, safe and age appropriate toys are provided \342\200\242 Current roster is NOT on file. However, no children enrolled at this time or for the last 5 years per the licensee. SEE LIC9102TV for Advisory Note issued. This is not a deficiency. \342\200\242 Notification of Parent\342\200\231s Rights poster is posted \342\200\242 Documentation of fire and disaster drills are to be conducted at least every 6 months once children are present and enrolled at the facility. \342\200\242 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \342\200\242 Verification of control of property is on file \342\200\242 Children\342\200\231s records are complete \342\200\242 Employee\342\200\231s records are NOT complete. \342\200\242 Mandated Reporter Training completed but is EXPIRED. This is not a deficiency. \342\200\242 Pediatric CPR and First Aid Card EXPIRED. However, no children enrolled at this time or for the last 5 years per the licensee. This is not a deficiency. \342\200\242 Health & Safety Certificate - completed and on file \342\200\242 Resident and/or staff records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions. Licensee [or facility representative] was reminded that all adults 18 and over, living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. One can add their email address and choose which program(s) they wish to receive Provider Information Notices (PIN) for. https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe The Duty Officer is available to answer questions Monday \342\200\223 Friday; 8:00am to 5:00pm at: 1-844-LET-US-NO (1-844-538-8766) and/or 951-782-4200 Facility was provided the following on today's visit: - A list of the items needed to be posted which includes: LIC610A \342\200\223 Emergency Disaster Plan, PUB394 \342\200\223 Notification of Parent\342\200\231s Rights, LIC9148 \342\200\223 Earthquake Preparedness Checklist, LIC999A \342\200\223 Facility Sketch, Complaint Hotline (1-844-LET-US-NO), Emergency/Disaster drill log, CA Child Passenger Safety Law, and License. - A list of items needed for an assistant and for adults that reside in the home. - CDPH 286, to use for future children's files. - LIC9040 Roster, to use for future children enrollment. - A letter regarding licensing fees that are past due dated 8/25/2025. Licensing is still waiting on receipt of recent annual fees that are overdue. Licensee provided proof that the check to fulfill this requirement was sent on 8/27/2025. There are no deficiencies being cited at this time, as there is no risk to children since there are no children enrolled at the facility. However, items listed on the LIC9102TV and LIC9102TA need to be corrected before childcare that complies with Title 22 regulations is provided. Licensee agrees to stay in contact with LPA to ensure they are ready to provide child care again. During the exit interview, the Licensee Gloria Guzman, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and this report was reviewed with the licensee Gloria Guzman. Appeal rights were discussed and provided during the exit interview.
