Based on an interview with the Licensee, the licensee did not comply with the section cited above when the Licensee did not report that the winds took down the fence that surround the in ground pool. This incident occurred two to three weeks ago. This is an unusual incident that should have been reported to the Department. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation and interviews, the licensee did not comply with the section cited above when the 2A10BC fire extinguisher that was purchased on 1/9/2020 has not been serviced or replaced. Therefore the fire extinguisher does not meet the State Fire Marshall standards, which poses/posed a potential health, safety or personal rights risk to persons in care.
