What the inspector wrote

On March 12, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on March 6, 2026. LPA met with Nadine Arvisu, facility representative who granted access and observed seventeen (17) preschool day care children and four staff in active care. On March 6, 2026, the reported information involved a child (C1) on the playground sitting on the edge of the play structure, released their hands, and fell backwards. Staff gave C1 an ice pack and took C1 to the nurse\342\200\231s office and the parents were notified. When the parent took C1 home, C1 stated they were pushed off the play structure by another child (C2). The parent returned to the school and requested to view video footage of the incident. During this inspection, LPA conducted interviews with all relevant parties involved. LPA conducted a safety inspection of the playground and play structure. LPA viewed the video footage and observed C1 and C2 were facing one another on the upper portion of the play structure then, C2 placed their hands on the bars above C1. It was not clear in the video footage if C1 were sitting or standing. The video revealed C1 fell backwards off the play structure without assistance. Staff were in the area, but the incident occurred suddenly and could not be avoided. Based on observation, interviews, and record review, the staff followed the proper protocol and administered aid to C1 and notified the parents in a timely manner. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, was given to Nadine Arvisu, representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
