This requirement was not met as evidenced by: Based on interviews, the licensee did not ensure, that S1s disciplinary action of pushing the broom bristles into the back of C1 knees, did not violate C1 personal rights. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

101216.3 (g) Teacher-Child Ratio: A teacher shall not be required to perform housekeeping or maintenance duties that prevent him/her from performing duties related to providing care and supervision to children. This requirement was not met as evidenced by: Based on interviews, teachers are responsible for cleanliness of the classroom. On August 12, 2024, an incident occurred due to S1 performing housekeeping duties rather than providing care and supervision. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

101538.3(b) Indoor Activity Space for School-Age Children: In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children. This requirement is not met as evidenced by: Based on LPA observation, the Licensee did not ensure the school age children were separate from the preschool children, when C1 was in a preschool classroom with 5 other preschool children at 3:45pm. This poses an immediate risk to the health and safety of persons in care.
