    Based on observation, the licensee did not comply with the section cited above when C1 opened the door to the Elephant classroom by self, entered the empty classroom and laid down on the carpet. When C1 engaged in these activities, neither S1 nor S2 were providing visual supervision to C1, which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on interviews, the licensee did not ensure each child was accorded dignity when S1 yelled and used the term “pissing me off” when speaking to C2 and other children in care. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

    Responsibility for Providing Care and Supervision 101229 (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met as evidenced by: Based on interviews, the licensee did not provide care and supervision to meet the needs of the children when C4 cut C5s hair without the staff being aware that C4 picked up the scissors in the classroom. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

On August 12, 2024, an incident occurred due to S1 performing housekeeping duties rather than providing care and supervision.     101538.3(b) Indoor Activity Space for School-Age Children: In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children. This requirement is not met as evidenced by: Based on LPA observation, the Licensee did not ensure the school age children were separate from the preschool children, when C1 was in a preschool classroom with 5 other preschool children at 3:45pm. This poses an immediate risk to the health and safety of persons in care.
