What the inspector wrote

On November 6, 2025, Licensing Program Analyst (LPA) Zirbes met with Facility Representative, Margaret Quintana to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self-reported unusual incident report (UIR) submitted to the Department on September 19, 2025. The unusual incident occurred on September 18, 2025 and involved Child 1 (C1), Child 2, (C2) and Child 3 (C3). Upon LPAs arrival today, there were four school age children with one teacher and one aide providing supervision. While LPA was on site, 15 additional school age children arrived to the Center. During this inspection, interviews were conducted with children and staff. In addition, LPA conducted a safety inspection, zero deficiencies were observed. Based on LPAs confidential interviews inconsistent information was received regarding the incident. At this time the incident does not appear to have been the result of a violation of the Title 22 regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative Margaret Quintana.
