The evidence gathered during the investigation reveals conflicting information. Staff denies the allegations and LPA was not able to retrieve additional information from other individuals who are pertinent to the investigation. Therefore, LPA is unable to corroborate the allegation. Although, the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are deemed unsubstantiated at this time. Exit interview was conducted with Director, Charmaine Maligaspe. A Notice of Site Visit was issued and must be posted for 30 day. A copy of this report was provided to the Director.
