During the facility tour the LPA was informed the outdoor play area has not been utilized for approximately two weeks. Upon inspection, the equipment was observed to have webs, a collection of thick dust/dirt, jagged edged cement block wall.

During the facility the LPA observed open bowls of cereal with milk & spoon in them, a bowl of watermelon, an open applesauce with a spoon in it and a sippy cup with fresh milk in it but a thick ring of unknown grim on the inside of the cup.

Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, risk to the children in care. During interviews it was disclosed a child was accepted by the facility with knowledge he/she had vomited prior to drop off. Additionally, during the child was at the facility the child vomited two more times.

Based on document review and interview, the licensee did not comply with the section cited above. Infants were relocated to a preschool classroom without Department authorization/waiver. This poses/posed a potential health, safety, or personal rights risk to persons in care.

Staff-infant Ratio: There shall be a ratio of one teacher for every four infants in attendance.This requirement is not being met as evidenced by: Based on interviews conducted, it was found that on 08/09, 08/14 and 08/15/2023 the facility was found to be out of ratio in the morning between 6:30am and 7:30am. This poses an immediate health, safety or personal rights risk to persons in care.
