REPORTING REQUIREMENTS
Upon the occurrence...of any of the events specified in (d)(1) below, a report shall be made to the Department within the Department's next working day...In addition, a written report...shall be submitted to the Department within seven days...This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in that the facility did not report an incident on 7/15/2024 where a facility van damaged a fence and the door of classroom 104. This poses/posed a potential health, safety, or personal rights risk to persons in care.
