Due to conflicting information obtained about the alleged allegations, the evidence collected was not sufficient to substantiate or refute the above allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED. Appeal Rights issued and discussed with facility representative and their signature on this form acknowledges receipt of these rights. Exit interview conducted, and report was reviewed with the facility representative Yahaira Martinez. A notice of site visit was given to facility representative Yahaira Martinez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. This report must be made available to the public for 3 years. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
