What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to conduct a case management inspection. LPA was granted entry by facility representative Lydia Mena. LPA toured the facility, inside and out, and took census. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 7/29/2025. It indicates that Child 1 (C1) sustained an injury to their head when they fell. Facility records were reviewed, and pertinent parties were interviewed. Based on the information gathered, the facility acted appropriately within the scope of the UIR, and no violations were identified within the scope of the UIR . The facility responded appropriately to the situation by applying appropriate first aid to C1 and notifying an authorized representative of C1. The facility took the extra step to move a shelf over the item that exacerbated the injury to prevent any future accidents from occurring. Facility plans to remove the items that could exacerbate injuries if children fall in the room to prevent the possibility of future accidents. An exit interview was conducted and a copy of this report was provided to facility representative Pamela Fox. A notice of site visit was given to facility representative Pamela Fox and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
