Inspector’s narrative

What the inspector wrote

On 04/30/2025, Licensing Program Analyst (LPA) Raymond Moorehead, arrived at the facility for the purpose of conducting an inspection on a separate matter. LPA was greeted by facility staff and granted entry to tour the facility inside and out. LPA conducted a census of a total of 32 infants in care. Upon arrival to the facility's front office, LPA observed an infant whom was sleeping in the arms of a staff member. LPA was informed that the the infant was sleeping in the office in the arms of a staff member as per parent/authorized representative's request and the infant's comfort for napping. Facility was reminded that Title 22 Regulation 101430 E states that if an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible. Due to LPA observation, it was determined that this action was in violation of Title 22 Regulation 101430 E which is titled "Infant Care Activities". LPA also consulted with facility staff and discussed a plan of correction for this violation. As a result of what was observed, there was one Type B citation that was assessed during today's visit. See LIC 809-D. Exit interview was conducted and the report was reviewed with Director Shannon Garcia and Assistant Director Heather Burr. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
