What the inspector wrote

Based on interviews with pertinent parties, records reviewed, and interview disclosures, a child was left in his/her diaper for approximately thirty minutes and a rash a had developed According to interview disclosures, staff are required to check a child's diaper every two hours and staff are required to change a child every two hours whether a child has a wet or soiled diaper. Based on this information, the department has determined the preponderance of evidence standard has been met, the above allegation is found to be SUBSTANTIATED See LIC809D for cited deficiencies. Based on information obtained during this investigation and through interviews conducted, the review of pertinent documentation, and Interviews revealing conflicting information, the allegations are UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegation occurred.
