In addition, a written report containing the information specified in (d)(2) below shall be submitted to the DepartmentThis requirement is not met as evidenced by: Based on record review and interview, the licensee did not comply with the section cited above in that, there were 3 instances where a child bit another child and broke skin. These unusual injuries were not reported to the Department, which poses/posed a potential health, safety, or personal rights risk to persons in care.

    Based on observation and interview, the licensee did not comply with the section cited above in that there were hand soap containers that stated "keep out of reach of children," in the dispensers next to all of the sinks that the children use in the classrooms and bathrooms. This poses/posed a potential health, safety or personal rights risk to persons in care.

Supervision shall include visual observation.This requirement was not met as evidenced by: Based on interviews, a parent observed a child splashing in toilet water in the 2's classroom, resulting in the child not being properly supervised at all times.This poses an immediate health, safety or personal rights risk to persons in care.
