Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to deliver an amended annual inspection report. LPA met with facility representative Teresa Smith, took census and toured the facility. The amended report was reviewed with facility representative Margaret Van Duzer and a copy was provided to the facility. The annual inspection report that the amended report is replacing was dated 6/02/2025 for the license number 364846072 . The amended report includes one LIC809C page and one LIC809D page. These amended pages replace the original pages that were previously delivered on 6/02/2025. A notice of site visit was given to facility representative Margaret Van Duzer and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the facility representative Margaret Van Duzer. This report must be made available at the facility for 3 years for public review upon request.
