Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

REPORTING REQUIREMENTS
101212(d) Upon the occurrence… of any of the events specified in (d)(1)... a report shall be made to the Department... In addition, a written report containing the information specified in (d)(2) below shall be submitted to the DepartmentThis requirement is not met as evidenced by: Based on record review and interview, the licensee did not comply with the section cited above in that, there were 3 instances where a child bit another child and broke skin. These unusual injuries were not reported to the Department, which poses/posed a potential health, safety, or personal rights risk to persons in care.

101233 Personal Rights – (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations...This requirement is not met as evidenced by: Based on interview and record review, there were 3 child-to-child biting incidents that broke skin within the dates of 1/14/2026 – 1/20/2026. The facility has since implemented operational changes that have greatly reduced biting incidnetns. This poses/posed an immediate Personal Rights risk to the children in care.
