ALARCON-ENRIQUEZ FAMILY CHILD CARE

What Happened

Facility had multiple safe sleep violations with infants including missing sleep logs for one infant, improper bassinet setup with oversized mattress and loose sheet, and one infant in an unsafe sleep sack. Type A citations issued.

Observations

  • 1 infant missing 15-minute safe sleep logs (does not comply with Title 22 - infants 0-2 must be checked every 15 minutes with documentation)
  • 1 infant sleeping in bassinet with mattress that does not fit the bassinet and sheet that does not fit tightly (does not comply with Title 22 - mattress must fit crib/play yard correctly)
  • Bassinet does not have integrated mesh coverings approved per Title 22 regulations
  • 1 infant observed in sleep sack (swaddled) - does not comply with Title 22 (infants shall not be swaddled while in care); immediately removed upon notification
