Pre-licensing visit·August 21, 2025

On the above noted date and time, Licensing Program Analyst (LPA), Diana Brasel conducted an announced Pre-Licensing inspection for to add a toddler option to their existing preschool license, facility number 364845734. Once approved the facility will become a single licensed facility. Upon arrival, LPA met with Licensee Yanping Zhang and Director Laura Perez. Licensee has requested a capacity of (12) toddler option children ages 18-36 months and (22) preschool children ages 2-5. Upon completion of measuring rooms 1 and 2, room 2 will be for toddler option capacity of (12) children and room 1 will be (21) preschool children. The LIC 200A was updated to reflect the change to preschool capacity. Hours of operation will be Monday - Friday 6:30 am - 6:00 pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Yanping Zhang and Laura Perez that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, director stated that they will be stored in large box on top of refrigerator. A fully equipped first aid kit is located in room 1 and 2. There is an operational carbon monoxide detector on site located in room 2. All required licensing documents were observed posted in the front entrance area. Children will be signed in and out at front entrance area. Fire clearance was granted on 6/18/25. LPA observed a total of (1) sink and (1) toilet available for toddler option children\342\200\231s use. LPA observed a total (2) sinks and (2) toilets for preschool children's use. These are sufficient to accommodate the requested capacity of (12) toddler option children and (21) preschool children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the directors area. LPA continued to tour the facility and measured all indoor and activity space. Total indoor activity space measured 421.55 for the toddler option children and 739.04 for preschool children, which is sufficient to accommodate the requested capacity of (12) toddler option children and (21) preschool children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via igloo filtered water and children's brought water bottles, disposable cups are available if needed. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Facility will provide breakfast, lunch and pm snack. The kitchen area currently includes refrigerator, freezer, sink, industrial warmer, and microwave. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via high cabinets. The facility currently has a fully fenced playground area. Fencing is a brink block wall, chain link and vinyl fencing that is at least four feet high. The total square footage for all the outdoor activity space is 949.44 for the toddler option playground and 2408.00 for the preschool playground, which is sufficient to accommodate the requested capacity. Shade is provided via large canopy on the preschool playground and an easy up for the toddler option playground (to be purchased see correction needed) There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is an age appropriate climbing structure on the preschool playground, which is properly anchored. There is adequate cushioning in fall zones of climber provided by cushioning material with indoor outdoor carpet. (see correction needed for other area of playground ground covering material) Drinking water is available from filtered water via children's water bottle and paper cups if needed. LPA observed all hazardous items on the playground to be inaccessible to children. Licensee and director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The licensee has stated she will purchase 1 or 2 high chairs for the toddler option component, the high chairs will be made of washable, moisture-resistant material. (see correction needed) Changing table have at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is NOT within arm\342\200\231s reach of a sink. (see correction needed). Diapering sink will not be used for meal preparation or dishwashing. Sleep logs are kept in room 2. Needs and Services Plans and Sleep Plans are kept in room 2. There is at least one potty chair or toilet for every five infants. Licensee and director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee currently does not provide IMS: \342\200\234Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Safe Sleep (for infant centers)\342\200\223 Child Care Centers and Family Child Care Homes: LPA discussed the safe sleep regulations with licensee and director. LPA also informed licensee and director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. MyChildCarePlan.org--Child Care Centers and Family Child Care Home: Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.\342\200\235 If the facility has had a lead test completed within the last five years from the pre-licensing visit date (06/30/2023) and is using the same outlets, you may submit a waiver request from the lead testing requirement to the Regional Office. The following corrections are needed prior to the issuance of the license: Toddler Option: 1. Proof that an adult sink within arms reach of the changing table with hot/cold water has been installed, shall be submitted. 2. Proof that the large plastic stand storing the mats has been anchored shall be submitted. 3. Proof of purchase of one high chair, shall be submitted. 4. Proof that the two small pink cubby drawer wooden cabinets have been anchored or removed, shall be submitted. 5. Proof that the outdoor fence dividing the two playgrounds measures 4 feet tall, currently it measures 3'1", shall be submitted. 6. Proof of a children's table and shade have been added to the toddler option playground, shall be submitted. 7. Proof that the divider set up in the restroom to separate the preschool toilets from the toddler option toilet has been anchored and the toddler option toilet area has been identified with a sign, shall be submitted. Preschool playground: 1. Proof that the material on the ground has been stretched or replaced and is no longer a tripping hazard, shall be submitted. 2. Proof that the wooden poles of the shade are with the peeling material has been cleaned or replaced, shall be submitted. LPA obtained all requested documents at time of visit, the documents will be reviewed. Licensee Yanping Zhang understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Perez.
