What the inspector wrote

This report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & the licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from the parent/guardian & place it in each child's file. If these requirements are not met, civil penalties will be assessed. An exit interview was conducted, and a copy of this report was provided to the Assistant Director Michelle Granados along with a Notice of Site Visit and Appeal Rights.

    Personal Rights(a) The licensee shall ensure that each child is accorded the following personal rights:(2) To be accorded safe, healthful, and comfortable accommodations, furnishings, and equipment to meet his/her needs.(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature including but not limited to interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by:Based on observation, interviews, and record review, it was revealed that the staff does not ensure C#1 has appropriate clothing while in care. Staff left C#1 walking without a shoe which resulted in C#1 having a blister under the foot. while in care which poses an immediate risk to the health and safety of children in care.
