What the inspector wrote

Staff interviews confirmed they are aware of child bitings taking place. LPA reviewed staff and child attendance records from 9/22/25 – 9/29/25. The investigation reveals that after reviewing sign-in sheets, children roster and staff attendance that the center had the staff to provide adequate supervision. Staff and child attendance records revealed that on each date the center was within ratio. During the complaint follow-up of 10/07/25, 11/20/25, and 12/23/25, the center was observed to be within ratio. During parent interviews, the parents confirmed to LPA that the facility provides digital and verbal communication for incidents and updated information regarding children to legal guardians. Based on information received, LPA confirmed on multiple occasions children encountered biting incidents. Staff have failed to correct or prevent the incidents for continuing to happen. This proves neglect or lack of supervision. Biting behaviors are documented on the Bright Wheel App. platform but has the center has failed to informed the department of such incidents. Allegation #1: Based on LPA's observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . Staff and Parent Interviews reported that the Center provides digital, verbal, and/or written reports regarding the child, and that at times children’s diaper needs have not been met. Interviews and digital records proof that at times there is lack of communication when it comes to diapers counts, children are sent home with the wrong and obtain diaper rashes because of it. Allegation #2: Based on LPA's observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . A Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with Assistant Director Marta Bizarron.
