What the inspector wrote

On 4/8/2024 @ 9:30AM, Licensing Program Analyst (LPA) conducted an unannounced inspection in reference to an allegation that the facility was operating out of ratio. LPA met with licensee, Pam Crisman. LPA toured the classrooms with Marcela Sison, Admin. Staff. At 9:30AM, LPA observed a total of 33 preschool children. LPA observed Room #1 with 17 children and staff Janet Dela Cruz. Staff Leila Tobias was observed in the adjoining room on the telephone and not actively supervising children. Based on LPA's observation and interviews with staff, it was determined that the preponderance of evidence has been met. There is enough supporting information to prove the above allegations are SUBSTANTIATED, see deficiencies cited on the attached LIC 9099D. Exit interview was conducted with Pam Crisman. A copy of this report, appeal rights and notice of site visit were given. Notice of Site shall be posted for 30 day.

THIS REQUIREMENT WAS NOT MET AS EVIDENCED BY:Ms. Crisman failed to notify the department when she designated the new designated site director in 2023.

    STAFF-CHILD RATIOThere shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance...THIS REQUIREMENT WAS NOT MET AS EVIDENCED BY: Based on LPA's observation, Room #1 was out of ratio at 9:30AM, there was one teacher supervising 17 children when the other teacher was observed in the adjoining room on the telephone and not actively supervising children.
