What the inspector wrote

Page 2 Interviews conducted and evidence collected support that on October 24, 2025 the CCC authorized a pickup that conflicted with the court order. Therefore, based on interviews conducted and documents gathered, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Deficiency is being issued. See 9099 D Page An exit interview was conducted, and this report was reviewed with Facility Representative Patricia LiBrizzi, Appeal rights were discussed and provided during the exit interview. A notice of site visit was given and licensee understands this must remain posted for 30 days.
