CHILD'S MEDICAL ASSESSMENTS

Based on interview and record review, the licensee did not comply with the section cited above where during file review, C4 file was missing a complete written medical assessment which poses/posed a potential health, safety or personal rights risk to persons in care.

OUTDOOR ACTIVITY SPACE

Based on observation,, the licensee did not comply with the section cited above where during the walk through, LPA observed a large ladder in the childerns outdoor activity space, DIR removed during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

101218.1(b)(6) Admission Procedures and Parental and Authorized Representative's Rights(b) At the time of acceptance of each child in care, the licensee shall inform each child's parent or authorized representative of his/her rights that include, but are not limited to, the following:(6) To request in writing that a parent not be allowed to visit a child or take a child from the child care center provided the custodial parent has shown a certified copy of a court order pursuant to Health and Safety Code Section 1596.857.This requirement was not met as evidenced by; Based on interviews conducted and evidence gathered, on October 24, 2025 the CCC allowed a pick up that conflicted with a court order, which poses/posed a potential health, safety or personal rights risk to persons in care.

REPORTING REQUIREMENTS

101212(d) Reporting RequirementsUpon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. This requirment was not met as evidenced by; Based on interview and record review, the CCC did not meet this regualtion in 3/3 recent unusual incidents occuring at the CCC within the last 12 months were not reported to CCLD.

1596.8595(d)(2) Posting licensing report by child care facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services(2) Upon enrollment of a new child in a facility, the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing document that the licensee has received during the prior 12-month period that pertains to a conference conducted by a local licensing agency management representative with the licensee in which issues of noncompliance are discussed.This requirement has not been met as evidenced by; Based on interview and record review, the licensee did not comply with the section cited above where the CCC could not provide evidence that newly enrolled families/children have been provided copies of any licensing document that the licensee has received during the prior 12-month period resulting in a Type A deficiency, which poses/posed a potential health, safety or personal rights risk to persons in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary...(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual ... This requirement was not met as evidenced by: Based on interviews during play yard time, a child wandered into an off-limit area of the building which is not part of the licensed facility without the knowledge of the staff.

CHILD'S MEDICAL ASSESSMENTS

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 persons were missing their TB clearance which poses/posed a potential health, safety or personal rights risk to persons in care.

PERSONAL RIGHTS

(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement was not met as evidenced by: Based on interviews revealing that S2 restrained C1 for approximately 15-30 minutes, which poses an immediate health, safety and/or personal rights risk to children in care.

(f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement was not met as evidenced by: S1 did not inform C1's parents of an unusual incident invlolving C1, which poses a potential health, safety, and/or personal rights risk to children in care.

Based on record review, the licensee did not comply with the section cited above as one staff (Ref#4) was missing docuementation for measles immunizations which poses a potential health, safety or personal rights risk to persons in care.
