What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived unannounced at the facility and met with Site Director Jesus Mendoza to conduct a plan of correction visit, from deficiency issued on October 30, 2025, with a plan of correction date of November 07, 2025. As of the date and time of this visit, the Plan of Correction for deficiency CCR 101218.1(b)(6) has been met as evidenced by: During the tour of the facility, LPA Gerth discussed the POC documents submitted to CCL, with Site Director Jesus Mendoza, and found that the documents reflected the agreed upon plan of correction for the deficiency issued. An exit interview was conducted with Site Director Jesus Mendoza. Appeal Rights were discussed and provided to Director, A Notice of Site visit was given and Site Director Jesus Mendoza was reminded the notice must remain posted for 30 days.
