What the inspector wrote

On 04/29/26, Licensing Program Analyst (LPA), Kelli Waters, conducted an unannounced Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted to Licensing by the facility on 04/15/26, regarding an incident that took place on 04/07/26. LPA met with Site Supervisor, Jesus Mendoza, to discuss the incident. The Site Supervisor reported the following: on 04/07/26, a parent called with a concern that their child (C1) had told them that a staff member had grabbed their arm, pulled them and scratched their arm. During the investigation, LPA Waters interviewed staff, reviewed records, and viewed video footage of the incident. Video footage revealed that a staff member (S1) grabbed a child (C1) in a rough manner by their right arm, moved C1 across the room by their underarms, and left a mark on their arm. After parent of C1 made the facility aware of the incident, video footage was reviewed and facility took action against staff. Site Supervisor confirmed that staff training on children’s Personal Rights was conducted in January of 2026 and will have additional training in May of 2026. Based on the information obtained during the visit, LPA Waters determined that the center did not ensure the Personal Rights of the child under Title 22 Code of Regulations Personal Rights 101223(a)(3). A type B citation will be issued. An exit interview was conducted, and a copy of this report was provided. A copy of this report must be made available to the public, at the facility site, for 3 years.
