101238(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by; Based on observation, LPA observed stacked furniture, 2 wooden pallets, push carts and storage bins with building materials on top, accessible gardening tools, a ladder, and large stacked boxes accessible to children in the outdoor activity space, which poses a potential health, safety or personal rights risk to persons in care.

This is a potential risk to the healthand safety of children in care.
