What the inspector wrote

An exit interview was conducted, and this report was reviewed with Director Holthaus. A copy of this report as well as a Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) were provided to Ms. Holthaus and her signature on this form acknowledges receipt of these rights. LPA observed Notice of Site Visit being posted. Notice of Site visit must remain posted at the facility for 30 days. An exit interview was conducted with Director Holthaus. The director was provided with a copy of this report and her appeal rights (LIC 9058) and her signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed Ms. Holthaus post notice of site visit.

    101223(a)(3) Personal Rights:(a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement was not met as evidenced by: Based on interviews conducted by LPA, S1 picked up a child (C1) by one arm and one leg to move the child while upset with the child and S2 grabbed/pulled another child (C2) by the arm/wrist to move the child. This poses a potential health, safety or personal rights risk to the children in care.
