    101212(d) Reporting Requirements - (d)...during the operation of the child care center...a report shall be made to the Department by telephone or fax...submitted to the Department within seven days following the occurrence of such event. This requirement was not met by: Based on file review, the licensee did not call or fax the Department to report the unusual incident that occured on 7/15/24. The Department was notified on 7/19/24 and therefore posed a potential risk to the health and safety of children in care.

    101223(a)(3) Personal Rights:(a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement was not met as evidenced by: Based on interviews conducted by LPA, S1 picked up a child (C1) by one arm and one leg to move the child while upset with the child and S2 grabbed/pulled another child (C2) by the arm/wrist to move the child. This poses a potential health, safety or personal rights risk to the children in care.

    101226.3 Observation of the Child: (b) Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. This requirement was not met as evidenced by: Based on interviews conducted by LPA and record review, on 6/29/23 a daycare child (C1) had an allergic reaction to a bee sting and was transported to the hospital emergency room. A record of this incident is not recorded in the child's record which is kept at the facility. This poses a potential health, safety, or personal rights risk to the children in care.

    101226 Health-Related Services: (a) The licensee shall immediately notify the child's authorized representative if the child becomes ill or sustains an injury more serious than a minor cut or scratch. The licensee shall obtain specific instructions from the authorized representative regarding action to be taken. This requirement was not met as evidenced by: Based on interviews conducted by LPA, on 1/18/23 the parent/guardian of a daycare child (C1) was not immediately notified that the child was ill & vomiting in a classroom. This poses a potential health and safety risk to the children in care.
