What the inspector wrote

On 12/11/25 Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced Case Management-Incident visit to follow up on two self-reported unusual incidents that were received by the Department on 11/7/25 and 12/5/25. Upon arrival, LPA met with Program Director, Tamara Holthaus. There were 83 preschool children, and 17 staff members present today. The facility reported that on 11/7/25 and 12/5/2025, Child #1 told their parent that Child #2 touched them in an inappropriate manner. LPA reviewed pertinent files and spoke with staff and parents. Neither incident was observed by staff. There was also a college student child observer that had been observing Child #2 since September that also has not observed the behavior or anything similar to the incidents reported at the facility. There was not enough proof of this type of behavior as to whether it occurred. Therefore, no deficiencies were cited today. Exit interview was conducted and report was reviewed with the Program Director, Tamara Holthaus. A notice of site visit was posted and must remain for 30 days.

This requirement was not met as evidenced by: Based on file review, staff interviews, and parent interviews there are children biting, hitting, and exposing themselves to others repeatedly. This poses/posed a potential health, safety or personal rights risk to the children in care.
