What the inspector wrote

On 03/21/25, Licensing Program Analysts (LPA) Kelli Waters arrived unannounced to follow up on a Unusual Incident Report (UIR) submitted to Community Care Licensing (CCL) on 03/04/25, regarding an incident that took place on 03/03/25. LPA met with Director Nialea Ortega-Guachena, and informed her of the reason for the visit. LPA conducted a facility inspection, took census of the facility, reviewed records, gathered documents, and conducted an interview. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided. Exit interview conducted and report was reviewed with the Director Nialea Ortega-Guachena. Appeal rights were provided and a Notice of Site Visit was given and must be posted for 30 days.
