Guardian database search conducted today, 2/21/2025 indicated that Bibi's fingerprint process was closed as the department's request for further documentation was not received. This deficiency poses an immediate health, safety or personal rights risk to persons in care.

Based on interview andrecord review, the licensee did not comply with the section cited above. Ms. Aryoubi admitted to not conducting a fire drill since she was licensed in May 2022. The fire drill log that was posted on licensee's wall was blank and did not contain dates of when drills were conducted. This deficiency poses a potential health, safety or personal rights risk to persons in care.
