What the inspector wrote

On 4/3//25 at 9:45 am Licensing Program Analyst (LPA) Gerald Poindexter was conducting a visit concerning another matter at the facility. LPA met with licensee Methak Alzerkany. Also, present and assisting with Arabic translation was the licensee's adult son Abdulla Alsudany. There was a total of 3 daycare children present. LPA discussed with Ms. Alzerkany the details surrounding a 3/5/25 injury incident at the facility and the regulation related to that matter. The incident involved a child falling to the floor from a high chair and sustaining an injury that required medical attention. Based on interviews with the Department, Ms. Alzerkany acknowledged that the child was partially and not completely strapped into the high chair, per manufacturers recommended guidance. See LIC 809D for deficiency cited. LPA Gerald Poindexter informed licensee that this report dated 4/3/25 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Poindexter informed the licensee to provide a copy of this licensing report dated 4/3/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the licensee, Methak Alzerkany. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply
