What the inspector wrote
On 9/3/2024 at 9:02AM, Licensing Program Analysts (LPAs) Nancy Diaz and Adriana Macias conducted an announced Pre-Licensing inspection with the applicant, Omega Asker. Bashar Butrus (husband) and daughter Ur Youssef were also present and helped translate in Arabic. Mrs. Asker resides in the home with her husband and 2 adult children - Azal Asker & Awj Isam.
This 4-bedroom, 2-bathroom single family home was toured and inspected to ensure an environment safe for the care and supervision of children.
The applicant owns the property and provided proof of control of property.
Applicant maintains smoke and carbon monoxide detectors and fire extinguisher in the home that meets regulation requirements. Both detectors were tested today and deemed to be operable. All hazardous items were latched and secured under the kitchen and bathroom sink and are out of reach of children. Mrs. Asker stated that she does not maintain any weapons. Applicant maintains a first aid kit in the home.
A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Control of property was provided to the department via copy of mortgage statement. First Aid and CPR will expire on June 2026 and Preventative Health Practices course was completed on July 14, 2024. Mandated Reporter Training was waived due to limited English proficiency of applicant. Mrs. Asker's primary language is Arabic.
The daycare children will have access to: family room, First bedroom to the right, hallway bathroom and patio area. Off limit areas are: living room, dining, kitchen, master bedroom and children's bedroom, backyard and pool area. Mrs. Asker was reminded that she must provide visual supervision when children use patio as the fencing is less than 4 ft.
LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted.
The new provider packet was also reviewed with the applicant including information on child abuse reporting, children\342\200\231s records, immunizations, adults living or working in the home, SIDS, Incidental Medical Services, and the YMCA Resource Center. Applicant was reminded that corporal punishment, smoking, walkers, exersaucers, jumpers and bouncy seats are not allowed in day care. Applicant has met all immunization requirements per SB792 and have completed the AB1207 Mandated Reported Training. Applicant is reminded to make anything that reads, "Keep Out of Reach of Children" inaccessible to children.
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Applicant states that she will comply with all regulations and laws governing family child care homes and that she is financially secure to operate a family child care home for children. LPA reviewed this report with Applicant prior to obtaining her signature.

as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was informed of the
myChildCarePlan.org
site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

On this date, 06/24/2024 the California Attorney General \342\200\223 Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California\342\200\231s Megan\342\200\231s Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
CORRECTIONS NEEDED PRIOR TO LICENSURE:
1. Install a 5-ft. fencing around the pool that is unclimbable with a self-latching gate.
2. Make cables inaccessible in the bedroom (where children will nap).
3. Install latch to the bedroom and hallway closets.
4. Make master and son's bedroom inaccessible to children.
5. Remove plywood from patio area.
6. Install barricade to make dining and kitchen inaccessible to children.
Corrections are due no later than September 30, 2024.
An exit interview was conducted with the applicant. A copy of this report and appeal rights were provided.
During the exit interview, the licensees, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
