Pre-licensing visit·October 16, 2024

On 10/16/2024 @ 12:55PM, Licensing Program Analyst (LPA) Nancy Diaz conducted an announced prelicensing inspection with applicant Omega Asker. Her husband Bashar Butrus Asker was also present. The purpose of the follow-up prelicensing inspection was to observe correction as requested on 10/3/2024. The following corrections were observed today: Applicant installed gates to doors that had direct access to the pool. The gates were 6 ft. high, swings outward and are self-latching. Door knob covers were installed to make master and son's bedroom inaccessible to children. Mrs. Asker was reminded that children must be supervised during outdoor play. She stated that the play structure will not be used by children and will be given away. A license for a small family child care is issued effective today, 10/16/2024. Exit interview was conducted with Mrs. Asker. LPA reviewed and provided a copy of this report to Mrs. Asker.
