What the inspector wrote

On February 7, 2023 at 9:00 a.m. Licensing Program Analysts (LPA\342\200\231s) Leilani Curtis and Samantha Clenista conducted an unannounced case management inspection in reference to the licensee\342\200\231s request for an increase in capacity and room addition. The licensee has requested to increase the facility capacity from 103 children to 175 children. The facility has requested to serve 118 preschoolers in preschool rooms #1, #2, #3, #4, #5 and #6, and 57 toddlers in toddler rooms #1, #2, #3 and #4. The facility is currently licensed to operate in preschool rooms #1, #2, #3 and #4 and toddler rooms #1 and #2. Upon arrival, LPA\342\200\231s met with Director Heather Davis and Licensee Saied Nakhai and proceeded to tour the facility. There was a total of 87 children present with 13 staff members. Appropriate ratios and capacity were observed. Staff members have the required background clearances and are associated to the facility. The following classrooms were measured today: Preschool Room #5 has 1,168.50 sq. ft. of indoor space which will accommodate 33 children. Preschool Room #6 has 1,107.70 sq. ft. of indoor space which will accommodate 31 children. Toddler Room #3 has 444.20 sq. ft. of indoor space which will accommodate 12 children. Toddler Room #4 has 497.74 sq. ft. of indoor space which will accommodate 14 children. The following classrooms were previously measured: Preschool Room #1 has 922.02 sq. ft. of indoor space which will accommodate 26 children. Preschool Room #2 has 758.36 sq. ft. of indoor space which will accommodate 21 children. Preschool Room #3 has 622.24 sq. ft. of indoor space which will accommodate 17 children. Preschool Room #4 has 163.63 sq. ft. of indoor space which will accommodate 4 children. Toddler Room #1 has 705.49 sq. ft. of indoor space which will accommodate 20 children. Toddler Room #2 has 458.57 sq. ft. of indoor space which will accommodate 13 children. The total square footage for all 6 preschool classrooms combined is 4,742.45 sq. ft. which will accommodate 135 children. The total square footage for all 4 toddler classrooms combined is 2,106 sq. ft. which will accommodate 60 children. The total indoor activity space for all children is 6,848.45 sq. ft. sufficient for a total of 195 children. The following outdoor play space was previously measured: Preschool Playground #1 has 6150.00 sq. ft. of activity space which will accommodate 82 children. Toddler Playground #1 has 1268.55 sq. ft. of activity space which will accommodate 16 children. The following additional outdoor play space was measured today: Preschool Playground #2 and side patio combined has 6,379.59 sq. ft. of activity space which will accommodate 85 children. Toddler Playground #2 has 838.27 sq. ft. of activity space which will accommodate 11 children. The total combined preschool outdoor activity space is 12,529.59 sq. ft. which will accommodate 167 preschool children at one time. The total combined toddler outdoor activity space is 2,106.82 sq. ft. which will accommodate 28 toddler children at one time. The fire clearance for the capacity increase and room additions was granted on 01/26/23. LPA\342\200\231s observed that preschool room #5 and toddler room #3 have age appropriate furnishings and equipment. These rooms were observed to be safe and free of hazards. Preschool room #6 and toddler room #4 have not been furnished and do not contain activities or equipment. The licensee states that preschool room #6 and toddler room #4 will not be used at this time. The licensee will send assigned LPA photographs of the furnished rooms for approval prior to use. The facility has a total of 28 sinks and 19 toilets which is enough to accommodate the requested capacity. LPA\342\200\231s also observed that the outdoor activity space (playgrounds) for preschool playground #2 and toddler playground #2 have not been completed. The licensee states that he anticipates that the playgrounds will be completed in approximately two weeks. There is shade available on the outdoor activity space. LPA\342\200\231s reminded Licensee that direct visual observation of all children indoors and outdoors shall be provided. Drinking water is available inside and outside of the classrooms. Student sign in/sign out is completed electronically via Procare and parents drop off children at the facility gate to be escorted to the classrooms by facility staff. The director states that snack is prepared in the classrooms. An operational carbon monoxide detector is located in room # 5. Prior to licensure, the following must be completed: Licensee stated he will submit a waiver request to allow the toddler outdoor play space to accommodate 28 toddlers at one time. The licensee will attach to the waiver request the class playground schedule. Licensee stated he will submit an updated facility sketch indicating off-limit areas, staff restrooms, number of toilets/sinks and placement of outdoor area safety gates. Licensee stated that he will place signs/labels on staff restroom door and off-limit areas. Licensee stated he will submit photographs of the completed Preschool #2 and Toddler #2 outdoor play areas/playgrounds. LPA will request for an updated fire clearance reflecting the capacity of 174 instead of 175 children. Licensee stated he will submit an updated rental agreement reflecting which areas of the property he is authorized to use. Upon approval of the outdoor activity space/playground waiver and receipt of the aforementioned items, a license for a capacity of 174 children (118 preschoolers and 56 toddlers due to limited outdoor activity space) may be granted. The licensed rooms will be as follows: Preschool Rooms #1, #2, #3, #4, #5 and #6, and Toddler Rooms #1, #2, #3 and #4. An updated license will be sent for posting. An exit interview was conducted with the licensee and appeal rights (LIC 9058) were discussed. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA\342\200\231s observed Licensee post notice of site visit.
