Inspector’s narrative

What the inspector wrote

On 3/27/25 at 8:00AM, Licensing Program Analyst (LPA) Annette Sutherland met with Heather Davis to address an unusual incident that had been self reported by the facility. The incident occurred on 2/27/25 which involved a child wandering away from the drop off area. The facility reported that the child managed to open the chain linked fence, pass through a gated area, descend a flight of stairs, to reach and push open the security fence. Staff member intercepted the child at the gate door below, preventing any harm from occurring. Upon further review, it was confirmed that the classroom was within the required staff-to-child ratio and was appropriately staffed at the time of the incident. However, the classroom staff did not immediately recognize that the child had left the group, which caused some initial confusion. Once the child was noticed on the stairs, staff acted promptly to address the situation. Since the incident the facility has installed a locking carabineer to the latch on the chain link fence. At this time no further action is needed. Appropriate action has been taken to prevent this from happening in the future. See LIC 809d for deficiency cited. Exit interview conducted and report was reviewed with the Director Heather Davis. A notice of site visit was given and must remain posted for 30 days.
