What the inspector wrote

On 07/24/2024 at 12:20pm, Licensing Program Analyst (LPA), Selina Siao conducted an announced Pre-Licensing inspection for adding an infant component to the licensed preschool facility 376701027 that has a toddler option component. Upon arrival, LPA met with Director Cynthia Bautista. Applicant is requesting to be licensed for 108 children including 12 infants ages 12 to 24 months in room D, 18 toddlers ages 18 to 36 months in room E and 78 preschool children ages 2 to kindergarten in room F, G, H, and I. Hours of operation is 7am to 6pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Director stated that they will be in the individual child's assigned classroom in a locked box and if it needs to be refrigerated then it will be in a locked box inside the refrigerator located in the kitchen/staff lounge area. A fully equipped first aid kit is in the high cabinet that is near the back door leading to the preschool large playground. There is an operational carbon monoxide detector on site located in the infant room D by the red fire alarm and in the kitchen on the wall near the entrance door. All required licensing documents were observed posted on the wall by the main entrance door and in the Director's office. Children will be signed in and out at the front by the entrance in a binder according to the child's assigned class. LPA continued to tour the facility all indoor and outdoor activity space. Total indoor activity space was previously measured to be 763.06 square feet for room D (infant component ages 12 to 24 months), 905.21 square feet for room E (toddler component ages 18 to 36 months) and 3190.12 square feet for room F, G, H, and I (preschool component ages 2 to kindergarten). Facility has sufficient indoor activity space to accommodate the requested capacity of 108 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via the brita water pitcher and the water is from the kitchen sink. LPA observed all hazardous items to be inaccessible to children. Director stated that there are no bodies of water or weapons on the property. Fire clearance was granted on 04/08/24. Changing tables have at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink in the Infant and Toddler room. Diapering sink is not used for meal preparation or dishwashing. Sleep logs are kept in a clip board for each child. Needs and Services Plans and Sleep Plans are kept in the Director's office. LPA observed a total of 1 sink in the infant room for the infants to wash their hands. There are 4 sinks and 3 toilets available for toddler children's use. There are 10 sinks and 6 toilets available for preschool children's use. These are sufficient to accommodate the requested capacity of 108 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill is the Director's office and the ill children will also use the staff's restroom. Facility provides snacks to the children in care. The kitchen area currently includes a stove, sink, microwave, and refrigerator. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in the kitchen are inaccessible to children by having the door closed. LPA advised facility to place a child safety door lock for the kitchen door to ensure children's safety. The facility currently has a fully fenced playground area which is separate from other components. The chain link fencing is at least four feet high. The total square footage for all the preschool outdoor activity space is 4800 square feet based on previous measurement and the toddler yard is 2048 square feet based on previous measurement. There is an approved outdoor waiver for the preschool children to be outside at different times in the AM and PM as it has sufficient outdoor space for up to 64 children. The toddler playground is sufficient for the requested capacity of 18 toddlers and 12 infants. Director submitted an outdoor playground waiver request for the infants to share space with the toddlers by schedule during today's inspection. Shade is provided via trees, patio cover, big blue shade awning cover for the upper and large preschool playground. And shade is provided with a sail shade, trees and building for the toddler/infant playground. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. There is a climbing structure on the large preschool playground that appears to be age appropriate for preschool children. There is adequate cushioning in fall zones of the climber provided by rubber cushioning and turf at the preschool outdoor area. The toddler playground has turf and rubber cushioning, and the play structure is for children 12 months and older. Drinking water is available via the children's own water bottle when the children are outside. Director stated the children's water bottles gets fill up with the brita water pitcher before outdoor play time. LPA observed all hazardous items on the playground to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Facility completed the lead testing in 09/18/2023 and it needs be retest every five years. The following corrections are needed prior to the issuance of the license: Obtain an approved outdoor playground waiver by the department for the infant to share space with the toddlers by scheduling.
