What the inspector wrote

On 10/2/25, Licensing Program Analyst (LPA), Saraliz Velando conducted an announced Case Management inspection for the purpose of changing the ages of the Infant Room. The Infant Room currently services Infants (12-24 months) in Room D and Licensee requested a change to service Infants (6-18 months) in Room D. The fire clearance was granted on 9/23/25. Upon arrival, LPA met with Director Cynthia Bautista. Licensee is requesting for the new license to remain at capacity of 108 children including 12 infants ages 6 to 18 months in room D, 18 toddlers ages 18 to 36 months in room E and 78 preschool children ages 2 to kindergarten in room F, G, H, and I. Hours of operation are 7am to 6pm. LPA measured the encumbered space in crib area and drop off area at the entrance of Room D. This area measured 121.28 sq. ft. and it was deducted from 763.06 sq ft which was measured previously on 7/24/24 by LPA Siao. LPA Velando observed 3 cribs in the crib area and the Director, Cynthia Bautista stated there is one more crib arriving soon. The crib area was observed to comfortably accommodate the 4 th crib. Total indoor capacity space is 641.78. in Room D. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. There is a changing table that has at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink in Room D. page 1 Diapering sink is not used for meal preparation or dishwashing. Sleep logs are kept on a clip board for each child. Needs and Services Plans and Sleep Plans are also kept in Room D. LPA observed a total of 1 sink in the infant room for the infants to wash their hands. The isolation area for children who are ill is the Director's office and the ill children will also use the staff's restroom. The Infant Yard is sufficient for the requested capacity of 12 infants. An updated license will be issued upon final review. No deficiencies were cited today. Exit interview was conducted and report was reviewed with the Director, Cynthia Bautista. Notice of site visit was given and must remain posted for 30 days. page 2
