What the inspector wrote

When it comes to allegation that facility is out of ratio , based on interviews conducted classroom always operates with an additional staff member to ensure ratio is being met and extra hands are available. Based on interviews and records reviewed when it comes to allegation staff are not following safe sleep regulations, it was revealed that staff complete the 15 minute sleep logs and ensure nothing is placed in the cribs with the infants. LPA observed 9 infants with 2 staff members during nap time, when LPA exited classroom the 3rd staff member was returning from lunch. LPA also observed sleep logs, sleeping plans and that cribs had no blankets or any items in them with the sleeping child. Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED . An exit interview was conducted, and this report was reviewed with the Facility Representative, Mersadies Henry , and a copy was provided. Appeal rights were discussed and provided during the exit interview. A Notice of Site visit was given, and Facility Representative understands that it must remain posted for 30 days.
