What the inspector wrote

Based on conflicting statements, LPA is unable to corroborate the allegation that a child sustained an injury while in care due to a lack of supervision. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is unsubstantiated. An exit interview was conducted, a copy of this report, along with appeal rights, and a Notice of Site Visit was provided to Director, Erica Webb. The Notice of Site Visit must remain posted for 30 consecutive days in a prominent place, visible to families and caregivers.

    Personal Rights:(a) The licensee shall ensure that each child is accorded the following personal rights:(2) To be accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs.This requirement was not met as evidenced by: Based on interviews and records reviewed, it was revealed that staff allowed infant aged children to go down a slide face first. This is not a safe or developmentally appropriate activity for infant aged children. This poses a potential health, safety, or personal rights risk to persons in care.
