What the inspector wrote

On December 20, 2024, at 2:45PM, Licensing Program Analyst (LPA), William Chancellor arrived unannounced to Children’s Choice Academy- Vista (CCC) and met with Director (DIR) Erica Webb. The reason for conducting a case management visit to address an issue separate from complaint investigation (Complaint Control Number 10-CC-20241018092403). During the complaint investigation, three of three staff interviews, corroborated that staff allowed infant aged children, including Child 1 (C1), to go down face first on the playground slide. Records revealed that on October 14, 2024, the date of the incident, C1 injuries were a result of going down the slide face first. While the original complaint allegation stated that C1’s injury was a result of lack of supervision, it was later corroborated that staff allowed the Elephant 1 classroom, consisting of children between the ages of one to two years old, to go down a slide, which is not developmentally appropriate. Based on interviews and record review, a deficiency is being cited under California Code of Regulations Title 22, Section 101223(a)(2): Personal Rights. See 809-D for cited deficiencies. An exit interview was conducted, a copy of this report, along with appeal rights, and a Notice of Site Visit was provided to Director, Erica Webb. The Notice of Site Visit must remain posted for 30 consecutive days in a prominent place, visible to families and caregivers.

This is not a safe or developmentally appropriate activity for infant aged children. This poses a potential health, safety, or personal rights risk to persons in care.
