What the inspector wrote
On 7/2/2025, at 1:31pm Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Annual Required inspection. LPA met with Facility Representative, Celia Carrizosa and disclosed the purpose of the inspection. LPA accompanied by Facility Representative toured the indoor and outdoor of the facility. There were 71 children present with seven (7) teachers. During time of inspection, the facility was observed to be operating beyond the capacity and limitations as specified on the license. LPA observed a total of 14 school age children which exceeds the license capacity of 12 school age children. LPA and Facility Representative discussed maintaining the capacity and limitations as specified on the license. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve children in care. Days and hours of operation are Monday through Friday from 6:00am to 6:00pm.
Furniture and equipment are in good condition. Floors in the facility are clean and safe. Toilets and handwashing facilities are in safe and sanitary operating condition. Disinfectants, cleaning solutions, medications and other hazardous items were made inaccessible to children during time of inspection. Kitchen, food preparation and storage areas are clean, free of litter/rubbish. Solid waste storage containers have tight fitting covers and are in good repair.
Playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Areas around high climbing equipment and slides have cushioning material to absorb falls. Facility has smoke and carbon monoxide detectors that meet statutory requirements. The last fire drill was conducted and documented on 6/25/2025.

Facility Representative stated there are no swimming pools or other bodies of water on the premises. Facility Representative stated that there are no firearms or ammunition allowed or stored on the premises.
Facility provides breakfast, lunch and snack. Menus are posted weekly. Children bring their own water bottles and filtered drinking water is available at the facility.
Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. Facility uses electronic sign in/sign out procedures. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day.
LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screenings, immunization records for influenza, pertussis, measles, and Pediatric CPR/ First Aid certifications. Staff 1 (S1) did not have current mandated reporter training certification during time of inspection.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018,
requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in
PIN 21-21.1-CCP

Based on interviews and record review, one (1) Type A deficiency of the California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 809D. A Technical Violation was issued.
LPA
informed Facility Representative, Celia Carrizosa that this report dated 7/2/2025 documents one (1) Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed Facility Representative, Celia Carrizosa to provide a copy of this licensing report dated 7/2/2025 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA provided Facility Representative, Celia Carrizosa with form LIC 9224.

A Notice of Site Visit (LIC 9213) was given to Facility Representative, Celia Carrizosa and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with the Facility Representative, Celia Carrizosa.
