TEACHER-CHILD RATIO

101216.3 Teacher-Child Ratio(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above, in that LPA observed 13 children in the 2 year old classroom being supervised by one teacher which poses a potential, safety, or personal rights risk to persons in care.

101225 Transportation (b)The manufacturer's rated seating capacity of the vehicles shall not be exceeded.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above, in that 10 children were transported in a 9 passenger van by Celia Carrizosa exceeding the vehicle's seating capacity which poses an immediate, safety, or personal rights risk to persons in care.

101225 Transportation (d) All vehicle occupants shall be secured in an appropriate restraint system.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above, in that C1 was transported in a 9 passenger van by Celia Carrizosa without a seat belt which poses an immediate, safety, or personal rights risk to persons in care.

REPORTING REQUIREMENTS

Reporting Requirements (f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement is not met as evidenced by: Based on interview, the licensee did not comply with the section cited above, in that C1's authorized representative was not notified that on 9/19/25, C1 was transported to the facility in a 9 passenger van without a seat belt which poses an potential, safety, or personal rights risk to persons in care.

LIMITATIONS ON CAPACITY

Based on observation, interview, record review, the licensee did not comply with the section cited above in that LPA observed the facility operating beyond the capacity limitations specified on the license in that there were 14 school age children present during the time of inspection which poses an immediate health, safety or personal rights risk to persons in care.

101216.3(a) Teacher-Child RatioThere shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance...This has not been met as evidenced by: Based on Directors own admission, interviews conducted and records reviewed, the facillity did not comply with the section cited above by having one teacher supervising more than 12 children from 10/23/23 to 11/8/23, which poses an immediate Health, Safety, and Personal Rights risk to persons in care.

101215.1 (d) The child care center director,or the substitute director...shall be on the premises during the hours the center is in operation. (f) ... arrangements shall be made for a fully qualified teacher....to act as substitute...shall be aware of center operations...designated as an authorized person to correct operational deficiencies...This requriement was not met as evidenced by: LPA arrived at the facility at 7:45 AM and the Director was not on the premises at the time of this inspection. LPA asked for the substitute Director and LPA was advised the substitute Director was not available as well. Staff did not know who was in charge. Director arrived on or about 8:30 AM.

HEALTH-RELATED SERVICES

101226(e)(3)(A) Health Related Services (e) In centers where the licensee chooses to handle medications: Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician. This requirement was not met as evidenced by: Based on LPAs observation and director's admission facility staff did not maintain medication per the label's instructions. This poses a potential health and safety risk to children in care.
