What the inspector wrote

On 6/24/2025 at 11:30AM Licensing Program Analyst (LPA) Mahjoba Mohsini made an unannounced inspection in response to a self reported incident that occurred on 3/18/2025, in which Child was momentarily left outside during transition from outdoors to the classroom. LPA met with director Ana Guiterrez. LPA interviewed staff and director regarding the incident. On the day of the incident at approximately 11:05 AM, 6 children were transitioning from outdoor area to class with 2 staff members (S1 and S2). S1 and S2 entered the classroom and door was closed, unintentionally leaving child outside. Child was in view of other teachers and in a fully fenced playground. Staff S3 and S4 from the playground immediately notified S1 that Child had been left behind. S1 also realized at that time and immediately went to retrieve Child. S1 notified the incident to the director who then contacted district director and child's parents. To prevent future incidents, the district manager issued a warning letter to S1 and S2, reminded staff to use the Face-To-Name Transition sheet consistently. LPA advised monthly the facility to hold monthly staff meetings to review supervision procedures and reinforce training expectations with staff. Deficiency was cited. See LIC 809D. Exit interview was conducted. LPA reviewed the report with facility director Ana Gutierrez. A copy of this report and notice of site visit was given today. Notice of site visit must be posted for 30 days
