PERSONNEL REQUIREMENTS

Based on record review, the licensee did not comply with the section cited above. Mrs. Elizabeth Villalpando did not complete a course on Preventative Health Practices which poses a potential health, safety or personal rights risk to persons in care.

PERSONNEL RECORDS

102416.1(a) Personnel Records- Personnel records shall be maintained on each employee and shall contain the following information... This requirement was not met as evidenced by: Based on record review and interview with licensee, Alexis Villalpando staff records were incomplete, missing several forms including LIC 508, LIC 9052, and LIC 9108. This poses an potential health and safety risk to children in care.

PERSONNEL REQUIREMENTS

102416(c) Personnel Records- The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement was not met as evidenced by: Based on interview and record review, licensee Alexis, provided a copy of receipt for enrollment in CPR/ FA for herself and co-licensee. This poses a potential health and safety risk to childrne in care.

ARTICLE 2. Administration of Child Day Care Licensing

1596.8662(b)(1)... training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion ... a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training... and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not met as evidenced by: Based on record review and interview, staff did not complete Mandated Reporter training. This poses a potential health and safety risk to children in care.

PERSONNEL RECORDS

Based on interview, the licensee did not comply with the section cited above as she stated records were not availalbe on site for facility staff or children which poses a potential health, safety or personal rights risk to persons in care.

OPERATION OF A FAMILY CHILD CARE HOME

Based on interview with licensee, Alexis, the licensee did not comply with the section cited above, no records were availalbe to review at the child care for any staff or children which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above. LIC 610 was not posted and no proof was availalbe that disaster drills have been conducted at least every 6 months which poses a potential health, safety or personal rights risk to persons in care.
