OPERATION OF A FAMILY CHILD CARE HOME

Based on observation and interview, the licensee did not comply with the section cited above in that cleaning compounds, medications and other hazardous items were observed in the kitchen underneath the sink, under the coffee stand and in the back yard area accessible to children which poses a potential health, safety or personal rights risk to persons in care.

INFANT SAFE SLEEP

Based on observation and interview, the licensee did not comply with the section cited above in that a small stuff animal was observed in a crib with C1 which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in that a crib mobile and infant mirror was observed by LPA attached to the right side of crib which poses a potential health, safety or personal rights risk to persons in care.

INFANT SAFE SLEEP

Based on observation, interview and record review, the licensee did not comply with the section cited above in that 15 minute sleep checks for C1 were documented for 1/21/25 and 1/22/25 which poses a potential health, safety or personal rights risk to persons in care.

102425(c)(1)(2) INFANT SAFE SLEEP An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 month of age the provider has in care and maintained at the facility in the infant’s file...This plan shall be signed and dated by the infant’s authorized representative...The Individual Infant Sleeping Plan...shall be maintained in the infant’s file and shall be available to the Department for review.This requirement was not met as evidenced by, Based on record review and Licensee's admission she is not completing LIC 9227 for the infants under 12 month in her care.

102416.2(c)(1) In addition to the events specified H & S 1597.467(b)(1)(A) through (b)(1)(C), the licensee...report the following events to the Dept.: Any suspected child abuse or neglect,...in Penal Code Section 11165.6,...child in care, in addition to reporting requirements pursuant to PC Section 11166. This requirement is not met as evidenced by: Licensee’s admission, sometime between April 2021 & June 2021, licensee raised concerns with the responsible party regarding scratches and bite marks on three daycare children in care. Licensee failed to report suspected neglect/abuse to the Dept. This is a potential health & safety risk to children in care.
